The following two treatments have been clinically proven and approved by FDA to effectively treat androgenetic alopecia. Those are finasteride tablet and minoxidil solution.
Finasteride is the generic name for the brand name drugs Proscar 5 mg and Propecia 1 mg. Finasteride is not recommended for women. The drug inhibits the enzyme 5-alpha reductase, thereby inhibiting the production of prostate-harming, follicle killing DHT. It helps both preventing hair loss and triggering regrowth. Recommended dose is 1 mg daily for 6 months until the patients will see the result. However if the patients stop using it, the hair loss will come back again within 12 months. The report showed that taking finasteride 1 mg daily for 5 years, patients are able to see effects from the medicine in triggering regrowth. For side effects, men may experience transient sexual side effects including erectile and libido difficulties.
Minoxidil was first used in tablet form as a medicine to treat high blood pressure. Further research showed that applying a solution of minoxidil directly to the scalp could also stimulate hair growth.
The research showed that applying 1ml of 5% concentration of minoxidil twice daily for 48 weeks is more effective than using 2% concentration of minoxidil. So it is recommended that patients clean and dry their scalp before apply 1ml 5% concentration of minoxidil twice a day. The result will be noticed after 6 months of medication and patients may continue using it to keep the result. Stop using Minoxidil, the hair loss to come back in 4-6 months. Minoxidil in Thailand can be found in form of solution; drop and spray. Applying minoxidil may have side effect such as skin irritation, contact dermatitis, or face hair growth etc.
As mentioned that minoxidil can be found in form of tablet as it is a medicine to treat high blood pressure. Taking tablet is not recommended to stimulate hair growth. Using minoxidil should be under the supervision of the doctor to avoid unwanted effects.
FUE or follicular unit extraction, is a minimally invasive method used to harvest and transplant hair. The treatment removes individual follicular units directly from the patient's donor site, which are then replanted into the scalp. Good thing about FUT is that it allows a maximum of 3,500 grafts harvested and transplanted in just one hair transplant session depending on patient’s scalp condition though. The graft extraction process involved in FUT is doing under microscope which reduces follicular graft transection, resulting in less hair graft damage and high rate survivability. When compared to FUE, FUT is faster and less expensive. However, FUT hair transplantation will create a long strip scar and involves more people in a surgery including the surgeon and team of hair transplant specialists, about 6-8 people on average per patient in a session.
An outline is designed and marked by surgeon where the new hairline will be created. The dominant hair zone is selected (the width of the stripe is about 1-2 centimeters depending on how many hair grafts need. The hair is taped so after surgery the wound will be hidden.
The selected hairs are shaved and the outline of strip is marked.
The donor site is local anesthetise and tumescent fluid is injected to plump up the skin in order to avoid nerves, vessels and hair shafts damage.
After local anesthetisation, open technique donor harvesting is applied using 4x prismatic loupes. Start with superficial scoring. Then a pair of small double skin hooks are placed and gently pull in the opposite directions while cutting perpendicular using a single-bladed knife to between the hair follicles.
After strip is removed, lower edge Trichophytic closure is applied to minimize donor scar. Then donor site is closed by retention suture and superficial running stitches.
The donor strip is slivered into thinner slices. Grafts are prepared under 10 times magnification stereo-microscope. Small slivers are separated into each follicular unit (Graft dissection). Each follicular unit is grouped and submersed in chilled normal saline ready to transplant.
Prior to transplantation, nerve block to numb the transplanted area by injecting local anesthetics around the main nerve trunk just on the forehead above the eyebrow.
Tumescent fluid is injected to reduce bleeding, damage of vasculation and nerve prior making recipient slits.
Recipient incisions are created using 4x magnifier prismatic loupes. Coronal slits are made using tiny needles or micro-blade to imitate natural looking. Grafts insertion is carefully done under magnifier. Stick-and-place technique is applied at the end of procedure to add up the density. Patient may experience swelling or numb on forehead after surgery but recovery is spontaneous.
After surgery, patients can live like normal. Hair washing is possible on the next day and can wash daily as patient’s wish with no worries about inflammation or infection. Post-surgery, patients should carefully follow instructions. Patient can lie down on the back but elastic headband should be placed at forehead to prevent facial swelling for a while. For the first period, Transplanted hair is shed then after 3-4 transplanted hair begins to grow. So the final result will be shown in about 8-10 months after surgery.
